You have finished your Induction in the community with one of our appointed trained staff, now it is time to fill in the form below, your Induction form. Please take five minutes, if there is anything that you require clarification, do not hesitate to ask your manager.

Did you had a meeting with the manager about your job description? *

Have you been issued Job description, including specific tasks related to the specific service user/s, staff you will work with, service users person-centred care plan which entails all the care requirements, i.e., fluids and nutrition to follow, Infection control requirements to prevent illness to the service user? To work - a Person centred way following the care plan in place? *

Did you had an outline of your role, responsibilities, level of authority and work priorities? *

Do you understand the service user condition described on the care plan, what entails effective communication with the service user, and how important it is to always promote their privacy and dignity? *

Have you received our Staff handbook, which includes all policies and procedures? *

Check that there are no matters outstanding from the recruitment process, e.g. all pre-employment checks completed. *

Did you read, discuss and understand terms and conditions of employment: - Information about pay day. - Payment method. - Completion of time sheets - How to report sickness / absence giving the name and Tel. No of the person to be contacted, by what time and what information is needed. - Explained annual leave entitlement, where this is recorded and who approves this. - Explained the disciplinary and grievance procedures, including whistle blowing, bullying and harassment. - Explained work rules and codes of conduct. - Explained personal development. *

Did you check that all relevant personal information has been obtained. For example, who to call in an emergency. *

New employees know where the basic amenities are. e.g. lavatories, cloakrooms, tea/coffee making facilities and eating area In the community this is not provided, consent is required from a service user to use their Toilet should you have an emergency. *

Are you aware of all forms of abuse and can identify these, report to the manager in line and you have the right to follow up the outcomes of the investigation? *

Have you been informed of specific points about working for the setting, e.g. no smoking policy, dress code, hours of work, tea and lunch breaks? *

In a home or building, can you explain the emergency procedures, e.g. evacuation exits and procedure, the sound of the fire alarm if there is one? *

Have you been introduced to clients in the setting and company office staff/seniors, explained who they are and what their role is? *

Care docs system, telephones, logins, data protection: Care documents are at service users’ homes or Software app: Care app – Employee's (you) are not to share log in details or any content of the app software with individuals not involved with the care of the service users. (i.e. Key safe numbers -not to be share with anyone at all) Confirm you will comply with this matter. *

You know how to access all the equipment needed to do your job, i.e. the safe use of hoists, standing hoists, stair lifts using sit belts and all requirements to maintain service user/staff safety, the use of Zimmer frames, walking sticks and any walking aids at all times as per care plan? *

Do you know what it means and expected in regard to - duty of care? *

Accidents and Incidents: Do you know and will comply to immediately report to the manager in line any incidents or accidents at service users home when delivering care and if reported to you as happened previous to your arrival? To call 999 first - if the service user is seriously ill/injured, life at risk and follow instructions from the NHS team? Accidents and Incidents Forms – Do you know how to complete Accidents and Incident forms, or to report to managment this will be done immediately. Risks assessments Forms – Do you know what a risk assessment entails and if asked you can identify the risks present at service users homes and report to management immediately? *
